MISSION STATEMENT: The goal of ONO community – based referral and evaluation program to maintain and enhance the lives of people for a better quality of life and care.
PROGRAM DESCRIPTION: ONO is a community – based assessment, referral and supportive program that refers people suffering from hypertension or high blood pressure. The goal of ONO is to maintain and enhance the lives of those with hypertension and other health problems. This is accomplished by screening for high blood pressure, identifying those individuals at risk of developing high blood pressure or are hypertensive and providing referral services for treatment. ONO care team will assist each person and provide a referral services to ONO medical doctor or their primary care physician. ONO care team will also assist each individual identified of having high blood pressure to obtain appropriate health and community resources. We are committed to adding life to years and not years to life.
ONO CARE TEAM: The care team has special training in blood pressure screening, evaluation and referral services. The team consists of a physician, health educator, a licensed administrator, social worker and a nurse.
